Australia is actually become “disabled people friendly” in recent years much has been done to improve the accessibility to places of interest, National Parks etc. and the use public transport has improved considerably . To list facilities for the disabled in each individual National Park, place of interest etc. for would just be too long.
My suggestion is to plan a route beforehand that you would like to travel and then see what NP´s, points of interest, towns etc. that would cater for your needs.
